Before You Begin

Have the following ready and this will go quickly: your driver's license, vehicle registration, insurance card, and Social Security Number (for employment verification and tax reporting purposes only — encrypted and never stored in plain text). You will also need to upload a few clear photos of yourself and your vehicle.

Every field marked with an * is required. Optional fields help us match you to the right jobs faster — fill them in if you can. If you need to step away and come back, your browser may save your progress, but we recommend completing it in one session.

All dispatch coordination, delivery assignments, real-time tracking, and driver support at Motion Miles Logistics are handled through our driver app. Once activated, you will receive access — all communication, job assignments, and proof of delivery happen inside the app. Phone calls to dispatch are reserved for urgent situations only.

Healthcare Delivery Certification

Drivers with current HIPAA and Healthcare Bloodborne Pathogens certifications qualify for medical and healthcare deliveries — including medical supplies, lab specimens, pharmaceuticals, and healthcare equipment. This certification unlocks a specialized and growing pool of work, significantly expanding your earning potential. Obtaining and maintaining these certifications is the sole responsibility of the driver.

To be eligible for healthcare deliveries, both HIPAA and Healthcare Bloodborne Pathogens certifications are required. You are welcome to obtain your Bloodborne Pathogens certification and submit it at any time after activation.

Background Check & MVR Authorization

As part of your application to drive with Motion Miles Logistics, we are required by law to obtain a consumer report, including a criminal background check and Motor Vehicle Record (MVR) covering the past 3 years. This report will be obtained from Checkr, an FCRA-compliant consumer reporting agency.

If you have questions, contact us at driver@mm-staging.zeabur.app. Under the FCRA, you have the right to request a copy of your background check report.

W-9 Tax Information

Why We Collect This As a 1099 independent contractor, federal law requires Motion Miles Logistics to collect your tax information before your first payout. This information is used solely to issue your 1099-NEC at year end if you earn $600 or more. It is stored securely and never shared with third parties.
